Complaints Procedure

1. Can I file a complaint about a dental service I received?

Yes. If you have any concerns about the quality of the service provided, you have the right to file
a complaint. Every treatment at our clinic is covered by an appropriate guarantee, and we review
each complaint individually and with the utmost care.

2. How long after the procedure do I have to file a complaint?

It is best to file a complaint immediately after noticing the problem. For most dental services
(e.g., fillings, crowns, dentures), a warranty period of 6 to 24 months applies
—depending on the type of treatment.

3. How can I file a complaint?

You can file a complaint:
• in person at the clinic reception desk,
• by phone,
• or by email at: [DENTA-MED email address].
Please include the following in your complaint: your first and last name, the date of your visit, the doctor’s name, and a brief description of the issue.

4. Do I need to come in for a follow-up visit regarding my complaint?

Yes, in most cases a follow-up visit is necessary, during which the doctor will assess your condition
and suggest a course of action. This visit is free of charge.

5. What happens after I file a complaint?

Po zgłoszeniu reklamacji:
1. Umawiamy Państwa na wizytę kontrolną,
2. Lekarz ocenia stan leczonego miejsca,
3. Jeśli reklamacja zostanie uznana – wykonujemy niezbędne poprawki lub zabieg naprawczy bez
dodatkowych kosztów.
Każda reklamacja zostaje rozpatrzona w terminie do 14 dni (minimum 2 tygodnie) od momentu jej
złożenia.

6. Is every complaint automatically accepted?

No—each complaint is reviewed on a case-by-case basis. If the defect resulted from causes beyond
the doctor’s control (e.g., poor oral hygiene, failure to follow post-treatment instructions, mechanical
trauma), the complaint may not be upheld.

7. Is the repair covered by the warranty free of charge?

Yes, if the claim is approved, all corrective measures will be performed at no additional
cost to the patient.

8. How long does it take to process a complaint?

Complaints are processed within 14 days of being submitted. We will notify the
patient of our decision by phone or email.

9. Can I file a complaint about treatment provided by another doctor at the same clinic?

Yes, the complaint concerns the entire DENTA-MED practice as the entity providing the medical service,
regardless of the attending physician.

10. What should I do if I'm not sure whether the issue qualifies for a return?

We encourage you to contact the front desk or your attending physician—together we will assess the situation and advise you
on the best course of action.
